Hospital Case Management

BLUE FOLDER Project
RFGH's BLUE FOLDER Project enhances patient medication safety and education. Your discharge nurse will provide you with a special blue folder containing your current medication list, disease education, upcoming appointments, and discharge instructions. Bring this folder to all medical appointments, pharmacy visits, and hospital encounters.
