PHI stands for Protected Health Information. It includes any information about a patient’s health status, treatment, or payment for care that can be used to identify that person — such as names, dates of birth, diagnoses, treatment details, or medical record numbers. Because email is not a secure way to transmit this kind of information, please do not send PHI by email. If you need to share PHI with us, please send it by fax instead.
