Diabetes is a complex and serious disease and managing it every day can be challenging and sometimes overwhelming. To help you succeed in your diabetes journey, diabetes educators have developed seven key areas to focus on: Healthy Eating, Being Active, Monitoring, Taking Medication, Problem-Solving, Reducing Risks, and Healthy Coping. A diabetes educator can help you set priorities and coach you in these areas to achieve your desired outcome.

All of these topics are included in the RFGH Diabetes Self-management Education Program. Currently, education is provided individually in 1:1 sessions with a nurse certified in diabetes care and education, and with a nutritionist. If you want to gain the knowledge, confidence, and skills to take control of your diabetes, your health, and your life, ask your doctor for a referral to RFGH's Diabetes Self-Management Training Program.

A referral includes a private one-hour meeting with the RN instructor to discuss your personal needs and plan your program. A one-hour 1:1 meeting with a dietitian will also be scheduled to develop a meal plan, especially for you. Follow-up meetings are then available and tailored to your current needs. Call our Diabetes Education Office at 858-8216 for more information.

We work closely with Redington Medical Primary Care's Endocrinologist, Dr. Celeste Quianzon, and Melissa Covenant, FNP. We also work with most healthcare providers in our service area to provide comprehensive and coordinated care. We offer education and support, advanced diabetes therapeutics, and training in technologies such as insulin pumps and personal continuous glucose sensors.

Diabetes is a serious disease that can raise blood sugars to dangerous levels. Over time, these high blood sugars do damage throughout the body. However, this DOES NOT have to happen. Blood sugars can be controlled with some healthy lifestyle changes and if necessary, medications. With blood sugar levels controlled, the damage is minimized.

Come join us and see for yourself. Diabetes is a significant inconvenience and if not controlled a serious threat to your overall well-being and quality of life. However, you are not alone and you don`t have to struggle with diabetes without support. We welcome you and look forward to helping you conquer your diabetes journey.
