Admission Information

If you are a surgery patient or anticipating a hospital admission, you will be scheduled a time to come for your routine preoperative tests, usually one to two weeks before admission. The length of this interview depends on your surgical procedure, patient teaching and testing needed. This can take approximately 1 1/2 to 2 1/2 hours. If you are contacted by phone, the nurse will obtain your information at this time. We will need to speak with you several days prior to your scheduled procedure.

Parking:

There is a designated Day Surgery parking area located at the Main Entrance.
When you arrive, go directly to Admitting, located on the first floor lobby area. Admitting staff will finalize the details of your admission with you.

What to Bring:

  • A list of your prescribed medications
  • A list of your allergies
  • Your current health insurance information
  • Completed pre-op interview questionnaire (Printable PDF form)

Advance Directives

When you talk with the patient representative, you will be asked if you have an Advanced Directive, such as a living will or other instructions for your care if you are no longer able to decide for yourself. Current law requires that we ask this question and it is not cause for alarm. If you would like more information about Advance Directives we will be happy to help you and provide forms. You may also download information from this web site. If you already have an Advance Directive and we do not have a copy on file at our hospital, please bring a copy with you.

Insurance Pre-certification

Many insurance companies require pre-certification for a planned hospital stay. The Patient Representative will remind you to call your insurance company before you come to the hospital to get approval prior to your admission.

Admission Day

You will be notified one day before your admission what time you need to arrive. Please bring protective containers for eyeglasses, contact lenses and dentures. Please do not bring large sums of money or other valuables.
Follow the pre-op instructions given to you by the nurse and take only those medications as instructed. Bring any other specific equipment as instructed into the Day Surgery Unit (i.e. crutches, home CPAP machine) on the day of surgery.
We encourage you to bring family members with you while you are being prepared for surgery.
Your surgeon will visit you prior to your surgery.
All patients having a Day Surgery procedure must have a responsible adult (over 18 years of age) present at the time of discharge home.
If you have any questions please contact the Day Surgery Nursing Staff at 474-5121 ext. 2102 between the hours of 7:00 a.m. to 4:00 p.m. Monday through Friday.
