Outpatient Infusion and Injection Therapies are available at Redington-Fairview General Hospital to meet the needs and schedule of your patients. Licensed providers may make referral for treatments at our clinics.

Here you will find order forms for some of our standard therapies. When completing the form, there are specific instructions at the bottom of each for initiating that therapy. For instance, some treatments may be directed to our Oncology Clinic rather than our Infusion Clinic.

Treatment in the Infusion or Oncology clinics is not limited to these therapies. For any inquiries, or for assistance with these forms, please contact our Infusion Clinic staff at 207-858-2407.
